Milford High School and Milford Central Academy Athletics will be transitioning to Rank One, a new online platform for all athletic registration and required DIAA Physical forms for the 2026–2027 school year.
According to the district, as of May 4, 2026, paper DIAA physicals for 2026-27 will not be accepted at any offices. Effective May 5, 2026, all physicals and registrations must be submitted online through Rank One.
As a reminder, a new DIAA physical (PPE) form is required for all student-athletes each year. All eligibility forms, including details on concussion, cardiac arrest, heat policies, etc. must be completed electronically through Rank One. No paper registrations will be accepted.
Parents and guardians should create a Rank One parent account and add their student(s) using their name and date of birth. Once completed, all forms can be filled out and signed online. The third page of the students’ physical or medical card can be uploaded. Only one account is needed per family even if there are multiple student athletes.
